As required by the "North Carolina Regulations For Protection Against Radiation" rules .0203 Application: Registration: Radiation Machines: Facilities: and .0205 Application For Registration of Service, each registrant shall complete proper application to register radiation machines and facilities. Healing
Arts Application: To complete the application submit page 1
(general information) and page 2 listing all x-ray equipment to this
agency. If the facility has
mammography equipment (FDA certified, non-certified, stereotactic, cabinet
specimen units) must include page 3 (mammography equipment list) of the
application. Facilities that are mammography only must
complete Page 1 (general information) and page 3 (mammography equipment
list). The application(s) must include the date and
signature before this agency will issue a registration number for your
facility. Non-Healing
Arts Application: To complete the application submit page 1 (general
