License Termination
 

The termination of a Radioactive Materials or Accelerator License is normally a two step process:  (1) decom- missioning of all use areas (to include disposal or transfer) and (2) application to the agency to terminate the license.   Listed below are some general guidelines for licensee consideration when terminating a license:

SEALED SOURCES ONLY:
  If the license authorizes the use of sealed sources ONLY, then the licensee should submit the following information in support of termination:

1.  Copies of the last leak test on ALL sources possessed and used under the license to demonstrate that no source was leaking;
2.  "Certificate of Disposition" for ALL sources possessed and used under the license to demonstrate that all sources have been transferred to a licensee authorized for that type, form and quantity of radioactive material.  This should include an acknowledgement from the recipient that they are in possession of the radioactive material.

NORMAL FORM RADIOACTIVE MATERIAL:
  For licensees who use and possess normal form radioactive materials, follow the guidance for decommissioning of the license.  The licensee is urged to contact the agency PRIOR to beginning decommissioning for information on that process.  The results of the decommissioning survey(s) and a Certificate of Disposition MUST accompany the license termination request.

ACCELERATORS:
  Provide information indicating that the accelerator(s) have been transferred to an authorized recipient.
