How can I terminate my license?
You must notify the agency in writing of
your request to terminate your general license. You must provide written documentation of
proper transfer/disposal of all devices that were possessed under the license. You are
responsible for any fees associated with your license until you have notified the agency
in writing of your request to terminate the license, and have provided documentation of
the proper transfer of your devices.

Do I need to appoint an
individual responsible for maintaining my license and ensuring
that applicable regulations are followed?
Yes, you need to appoint a "responsible individual" that
has knowledge of, and the authority to take required actions to ensure compliance with the
appropriate regulations and requirements.

How can I dispose of generally licensable devices that I no
longer need?
In most cases, a generally licensed device can only be disposed by
transferring to a person that holds a specific license for the device. The most common
method of disposal is to return the device to the manufacturer. You must get a written
confirmation of receipt from the person or organization receiving your device. You must
also notify the agency of the transfer.

Can I move or relocate a
device from one facility I own to another facility I also own?
In most cases, you cannot transfer or move the gauge from
one physical location/address to another, without notifying the agency. Depending on the
type device you have, it might be necessary to have a manufacturers representative
assist you with the reinstallation of the device. In ALL cases,
you must notify the agency in writing of any relocations of the device.

The company is undergoing a change
of ownership. What information is required to be submitted?
The answer depends on the circumstances surrounding the

questions regarding the specifics of the ownership change and inform you of
